Immunization Records ( can be faxed from Drs office to 304-755-3843)
Dental Records (Dr can fax to 304-755-3843)
Health/Wellness Records if available (Dr can fax to 304-755-3843)
Copy of IEP/504 if applicable
HS - 18 Form (only if your child will require medication at school)
