- Student’s Birth Certificate (state seal)
- Student’s Social Security Card
- Parent/Guardian Driver’s Lic/ID
- 2 Proofs of Residence (utility bills, tax ticket, lease/rental agreement, etc)
- Immunization Records (current)
- Custody Documents (if applicable)
- Copy of IEP or 504 Plan (if applicable)
