Vaccine Clinic

Putnam County Schools will partner with Fruth Pharmacy to host a COVID-19 Vaccine, Booster (Pfizer and Moderna), and Flu Shot Clinic on Friday, November 19, 2021, at Hurricane High School from 4:00 pm to 6:00 pm. Pre-registration will not be required; no appointments necessary. Additionally, children ages 5 to 11 years are now eligible for COVID-19 vaccination and can receive the Pfizer pediatric COVID-19 vaccine on Friday at the upcoming vaccination clinic.

📌 Pfizer Pediatric COVID-19 Vaccine (ages 5-11)

📌 Pfizer and Moderna COVID-19 Vaccine Boosters (ages 18 and above)

📌 Flu Shots

Important Reminders:

✅ Present a copy of an insurance card when checking in at the vaccination clinic.

✅ If receiving a booster shot, please remember to bring your COVID-19 vaccine card.

✅ All students must complete a consent form, and a parent/guardian must be present.

📝 Note: Your school office has both student and adult consent forms to be completed before the vaccination clinic. The consent forms may also be obtained on the PCS website (located and linked on the home page bulletin board) at www.putnamschools.com.

COVID-19 vaccines are also available in many locations throughout our community. Check with your child’s healthcare provider, pharmacy, or county health department to see whether they offer pediatric COVID-19 vaccination.
